📋 Description

• The RN Telehealth Administrator on Call provides remote administrative, clinical triage, and care coordination support for the PHP Telehealth provider team and patients across healthcare settings including skilled nursing, assisted living, and independent living facilities.
• Day-to-day responsibilities include coordinating with telehealth providers, communicating with onsite nursing teams, overseeing provider queues, supporting technical issues, monitoring EMR for lab and imaging results, documenting encounters, recording data in CRM, connecting patients with in-house providers, triaging calls, providing health education, and ensuring excellent customer experience.
• CommunityCare Health Services, Inc. operates through its medical practice arm, Personalized Health Partners (PHP), delivering telehealth services to seniors and residents in multiple states with a focus on person-centered, high-touch care and interdisciplinary collaboration.
• The role offers opportunities to develop clinical coordination skills, gain experience in telehealth operations, contribute to process improvement projects, and work within a collaborative healthcare team focused on improving patient outcomes and satisfaction.

🎯 Requirements

• Valid RN or LPN license
• Willingness to obtain licensure in the 7 states served (Indiana, Ohio, Maryland, Virginia, West Virginia, and Kentucky)
• Excellent communication and critical thinking skills
• Good technology aptitude
