📋 Description
- • Collaborates with interdisciplinary teams to assess, plan, implement, and document psychosocial factors impacting patients’ illnesses and continuity of care in both inpatient and outpatient settings.
- • Completes and documents comprehensive psychosocial assessments and interventions for identified patient populations.
- • Provides crisis intervention and counseling to patients, families, and significant others to enhance coping mechanisms related to illness and palliative care needs.
- • Engages patients, families, and interdisciplinary teams in the discharge planning process to identify appropriate post-acute resources and community service follow-ups.
- • Delivers education to patients, families, and care teams regarding psychosocial factors associated with illness, disease progression, and end-of-life care.
- • Actively contributes to enhancing the role of social work within the hospital through expertise in palliative care and case management.
- • Manages multiple priorities independently while maintaining accurate documentation and compliance with regulatory requirements.
- • Applies motivational interviewing and advance care planning competencies to support patient-centered decision-making.
- • Demonstrates cultural competency in addressing diverse patient populations and tailoring interventions to individual needs.
- • Understands the impact of lifestyle factors on disease processes and integrates this knowledge into care planning.
- • Works collaboratively as a team member in a high-performing, flexible healthcare environment.
- • Maintains strong organizational and time management skills to ensure timely completion of assessments, interventions, and documentation.
- • Utilizes Microsoft Office products for documentation, communication, and reporting tasks.
- • Travels 50%–75% of the time between office and various work sites, including patient homes, clinics, and hospital units.
- • Lifts up to 10 pounds constantly and up to 25 pounds rarely as part of daily duties.
- • Adheres to HIPAA regulations, company privacy policies, and data security guidelines when handling protected health information.
- • Follows the company’s Code of Business Conduct and complies with all federal, state, and local legal and ethical standards.
- • Teaches and trains others regularly as part of professional responsibilities.
- • Performs duties in an office-based role with significant fieldwork requiring mobility across Pennsylvania.
