📋 Description

• The Social Worker MSW provides quality psychosocial care to patients and coordinates care with the medical team while applying the social work code of ethics, playing a vital role in supporting patient well-being and holistic health outcomes.
• Day to day, the role involves identifying patients with complex care needs, conducting psychosocial assessments, developing intervention plans, coordinating resources such as housing and transportation, advocating for patients, facilitating discharge planning, and serving as a liaison between patients, families, and healthcare providers.

• In this role, the individual can deepen expertise in clinical social work, strengthen interdisciplinary collaboration skills, lead patient advocacy efforts, and contribute to reducing hospital readmissions through proactive care coordination and resource facilitation.

🎯 Requirements

• Master’s degree in social work
• Licensed/Certified as Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), Certified Independent Social Worker (CISW), or Certified Social Worker (CSW)
• American Heart Association Healthcare Provider Basic Life Support (BLS) certification
