- • Confirms and interprets physician referrals to guide patient care decisions in accordance with Speech-Language Pathology regulations.
- • Performs comprehensive patient assessments considering developmental and chronological age, as well as medical diagnoses, to determine appropriate treatment plans.
- • Designs and implements individualized treatment programs in collaboration with patients and/or family members to achieve established clinical goals.
- • Coordinates patient care through written and verbal communication with patients, families, physicians, Clinical Team Leaders, and interdisciplinary healthcare team members.
- • Documents all patient care activities accurately in medical records and completes required forms in compliance with regulatory agencies and departmental policies.
- • Collaborates with clinical staff, support personnel, volunteers, and students to ensure efficient clinical operations and high-quality patient outcomes.
- • May supervise Speech-Language Pathology students in alignment with Virginia state licensure standards and federal regulations.
- • Orients new employees and provides guidance to volunteers, support staff, and contract personnel as needed.
- • Participates in departmental and professional activities including in-services, performance improvement initiatives, program development, new employee orientation, committee work, peer review, and staff meetings.
- • Applies knowledge of life span growth and development principles to assess patient status and deliver age-appropriate care.
- • Conducts patient care in diverse clinical settings including hospital, nursing home, outpatient facilities, and patient homes for Home Care assignments.
- • Travels daily between patient homes for Home Care roles, requiring reliable personal transportation.
- • May be required to work at other Carilion Clinic facilities to meet staffing needs, with transportation provided by the employee.
- • Demonstrates ability to perform physical tasks including lifting, pushing, pulling, bending, and walking, with patients potentially weighing over 200 pounds.
- • May be exposed to radiation from video fluoroscopic studies, combative patients, prolonged standing, and frequent environmental hazards including blood and bodily fluids, requiring strict adherence to standard precautions.
- • May conduct and interpret videofluoroscopies, including dictation and sign-off procedures, depending on assigned site.
- • Suggests cost containment initiatives within the department to support operational efficiency.
- • Maintains access to and handles sensitive patient information as essential to job performance.
- • Communicates effectively with internal and external stakeholders, providing problem-solving, consultation, and interpretation services within scope of licensure.
