📋 Description
- • Supervise, hire, motivate, and evaluate a team of care managers delivering Healthy Maternity (HM), High-Risk OB (HROB), and NICU case management programs to ensure consistent, high-quality clinical outcomes.
- • Monitor team productivity and performance against departmental goals, identifying areas for quality improvement and implementing updated clinical and operational processes.
- • Conduct quality audits of clinical services, review audit findings, and provide targeted feedback and training to staff on clinical protocols, technical accuracy, and compliance with evidence-based practices.
- • Coordinate clinical operations with internal and external stakeholders to ensure seamless delivery of care management services and alignment with program objectives.
- • Facilitate the implementation of new programs, policies, and procedures related to maternal and neonatal case management, ensuring adherence to organizational standards and regulatory requirements.
- • Provide real-time coaching and performance management to frontline care managers during periods of team growth and operational change, maintaining stability and high standards of care.
- • Ensure all team members comply with clinical documentation standards, utilize approved digital tools, and maintain proficiency in communication via digital channels, including typing and grammar proficiency.
- • Support onboarding and development of new staff, fostering a culture of accountability, continuous improvement, and adherence to AI-enabled workflows guided by established policies and templates.
- • Manage clinical workflow efficiency to reduce NICU readmissions and lower total cost of care while maintaining high member engagement rates among high-risk and high-cost populations.
- • Perform administrative duties including performance evaluations, scheduling, conflict resolution, and resource allocation to optimize team effectiveness and program sustainability.
- • Ensure compliance with all required immunizations and medical tests for clinical staff in accordance with organizational health and safety protocols.
- • Maintain proficiency in Microsoft Office Suite (Outlook, Word, Excel, PowerPoint) to manage reports, communications, and data tracking related to case management outcomes.
- • Demonstrate strong interpersonal, organizational, and communication skills to collaborate across departments and lead a diverse team in a fast-paced, evolving care environment.
- • Function effectively both independently with minimal supervision and as part of a collaborative team, sharing responsibility and accountability for program success.
- • Apply leadership expertise to translate strategic goals into actionable, frontline execution that drives measurable improvements in maternal and neonatal health outcomes.
