📋 Description

• • The Transition of Care Nurse plays a vital role in guiding members as they move from higher to lower levels of care, ensuring discharge plans are comprehensive, appropriate, and focused on member safety and continuity of care.
• • Day to day, the nurse provides telephonic or onsite services to designated hospitals to assist care coordination teams with discharge planning, assesses resource utilization and cost management, coordinates member needs, develops and implements treatment plans, and evaluates available resources to promote quality, cost-effective outcomes.
• • BlueCross BlueShield of Tennessee fosters a culture where innovation is encouraged, including the responsible use of AI-enabled tools to support everyday work, and although based in Chattanooga, TN, this is a fully remote role.
• • In this role, the nurse can develop expertise in care transitions, influence patient outcomes, utilize clinical expertise to make a meaningful impact, and gain experience in coordinating post-acute care across diverse settings such as ICU, SNF, LTACH, and inpatient rehabilitation.
